- Lab-specific: Quest ↔ Quest, LabCorp ↔ LabCorp
- Quest codes often start with “Q” (green)
- LabCorp codes are numeric (blue)
- Sites should not charge prepaid donors
- Quest: must be processed in FormFox

Test code guide
Your code unlocks the collection. Codes are lab-specific — and Quest still requires FormFox at the site.
Quest · FormFox required
If the collector is not using FormFox for a Quest order, the collection will fail — every time. Stop and ask them to open FormFox before you start.
Need a different lab?
Employment tests: contact your employer for a new code.
Self-purchased: refund the unused test, then buy the correct lab — best done while you’re still at a facility if needed.
