Quest codes only work at Quest. The collector must process your order in FormFox — or the collection will not work.

FormFox is mandatory

Quest orders only process correctly when the collection site uses FormFox. Wrong software = failed collection, wasted trip, and a refund path.

  • Ask before collection: “Are you processing this in FormFox?”
  • If they say no, use paper only, or another system — do not proceed
  • LabCorp uses LabCorp systems — never mix networks or codes
Need a refund?

Bring with you

  • Government photo ID
  • Quest test code (often starts with “Q” — green form)
  • Do not pay the site for prepaid LTS orders

If FormFox isn’t used

  • Collection will fail / not process correctly
  • Do not continue with the wrong system
  • Find another site or start a refund
